Nurse Visit RN
Job Status : Full-time, Tuesday - Saturday, 8am – 5pm
Sign-on Bonus: $3000.00 (Internal candidates that are currently employed here at NWHS are not eligible for sign-on bonus.)
Referral Bonus : $1000.00
NWHS is currently seeking a compassionate and proactive Nurse Visit RN to join our Patient-Centered Medical Home (PCMH) team, where you'll provide high-quality, independent nursing care visits that support prevention, chronic condition management, and care gap closure.
YOUR ROLE:
As our Nurse Visit RN, you’ll work side-by-side with providers, behavioral health professionals, medical assistants, and care coordinators to empower patients through education, access, and coordinated services that improve health outcomes.
SPECIFIC DUTIES:
+ Conduct independent nurse visits for services such as immunizations, medication education, wound care, diabetes coaching, contraceptive injections, ABI testing, SLUMS assessments, and more.
+ Collaborates with care team to ensure seamless and patient-centered care.
+ Provide personalized patient education to promote self-management of chronic conditions, proper use of medications and devices.
+ Ability to accurately observe, analyze and interpret general and technical data both verbal and written, and to accurately communicate this information.
+ Ability to recognizes abnormal and critical clinical findings and takes appropriate action including notifying provider as appropriate.
QUALIFICATIONS:
+ Current Oregon RN license.
+ Minimum of 1-2 years of clinical nursing experience, preferably in an ambulatory care, community health, or FQHC setting.
+ Has strong clinical assessment, problem-solving, and decision-making skills with sound clinical judgement.
+ Able to provide compassionate, culturally competent care delivery.
+ Current Basic Life Support for Healthcare Providers (BLS) training certificate consistent with AHA Guidelines for CPR/ECC, and covers breathing and cardiac emergencies - including CPR, AED, and obstructed airway - for adult, child, and infant patients.
+ Bilingual preferred but not required.

Northwest Human Services sites are eligible for various Provider Incentive Programs through the Oregon Office of Rural Health and National Health Service Corps loan repayment and scholar programs as well as the Rural Practitioner Tax Cred.
