Responsibilities

+ Assist in the delivery of primary health care services and patient care management.

+ Demonstrates competence in the skills necessary to carry out assigned duties.

+ Administer medications by all routes.

+ Apply Nursing Process (Assessment, Diagnosis, Planning, Intervention and Evaluation – within licensure guidelines)

+ Act as a liaison with patients, family and providers consistently demonstrating smooth and efficient telephone communication when covering provider call and scheduling patient appointments.

+ Obtain complete history regarding health problems according to protocols.

+ Document accurate information regarding health concerns and/or condition from patient or family member.

+ Telephone Triage by providing and documenting advice for home care of illness/condition; conducting follow-up phone calls regarding patient's health and progress as necessary.

+ Ability to assess urgency of condition for immediate attention regarding illness related calls and assign priority.

+ Provide patient education regarding health maintenance and medical illnesses following accepted Teach-back method per protocols and guidelines.

+ Provide appropriate and timely documentation in the patient’s electronic health record using standardized workflows and processes.

Qualifications

+ Graduate of an accredited program for Registered Nurses.

+ Current license to practice nursing in the State of Iowa/Illinois for Registered Nurse.

+ Mandatory Reporter certification

+ CPR certification.

+ Previous clinical experience in a medical office preferred.
