Job Summary and Qualifications

The licensed practical nurse (LPN) provides nursing care to assigned patients under the direct supervision of the registered nurse (RN) or other assigned supervisor and accepts delegation from the RN/assigned supervisor in meeting the needs of the patient/family.

Major Responsibilities:

• Collects data through observation and communicates information to assist the RN and/or provider in patient assessment and care planning.

• Participates as a member of the health care team and accepts delegation from the RN/assigned supervisor and provider in meeting needs of the patient/family.

• Delivers care in accordance with the patient care plan, policies and procedures of the organization and principles of relationship-based care.

• Contributes to the enhancement of group effectiveness by exchanging information, collaborating with other team members, and providing constructive feedback.

• Helps develop team cohesiveness by offering assistance to team members and aligns individual work efforts to meet work unit and organizational goals for service excellence.

• Completes assignments on time, contributing extra effort to meet patients', customers', and colleagues' needs. Provides supports by:

• Promoting consistent positive patient interactions that advance the agenda of unparalleled patient service

• Practicing and adhering to the “Code of Conduct” philosophy and “Mission and Value Statement”

• Performing other duties as assigned

Education & Experience

• Associate's degree Nursing Required • 1+ year(s) of experience in LPN nursing Required Or equivalent combination of education and/or experience Licenses, Certifications, & Training:

• LPN / LVN Required

• Basic Cardiac Life Support Required Knowledge, Skills, Abilities, Behaviors:

• Excellent communication skills; is skillful in mentoring and teaching; and participates on committees or projects. Required

• Functions as a productive team member Required

• Computer skills and experience with electronic medical record systems Required "
