What You’ll Do

+ Be available (in person, by phone, or electronically) 24/7 during operating hours as required by regulations

+ Oversee patient and staff assignments

+ Coordinate patient care and manage referrals with continuous assessment

+ Review quality assurance documentation for assigned records

+ Manage and monitor patient and clinician schedules

+ Stay current with OASIS data set requirements, home care reimbursement, and compliance

+ Develop expertise in agency software systems

+ Attend mandatory meetings and trainings as required

+ Actively participate in quality assurance and performance improvement initiatives

+ Conduct home visits and complete comprehensive/OASIS assessments as needed

+ Serve as the liaison between clients, physicians, and Phoenix Home Care

+ Participate in the on-call rotation as assigned
