As the Social Worker, you will be responsible for providing services to respond to the emotional needs of the residents and their families. You will meet with the resident and family to review policies on advance directives and resident rights. Conduct and document a social service evaluation, including identification of resident problems and needs. Maintain communications with caseworkers. Maintain a current list of community resources to facilitate referrals. Confidentiality is a must. Other duties as assigned.

Requirements:

Prefer current license/certification in the state of practice.

Must have a minimum of (2) years experience as a Social Worker in related field, hospital, nursing care facility, or other related medical facility.

Prefer (1) year experience in Long term care.

Must be able to read, write and understand the English language.
