    Summary The incumbent will be responsible for the professional and administrative management of the Mental Health Clinical Pharmacist Practitioner group within ECHCS. Responsibilities for supervising multiple pharmacists, including clinic grids & employee leave requests. Maintains interdepartmental relations & cooperates with other services to accomplish the ECHCS mission & goals. Develops & updates performance standards & completes annual & mid-year reviews. Develops policies & procedures as needed. Responsibilities Duties and skills needed to be successful in this position include, but are not limited to: Clinical - Direct Patient Care Activities: Functions as an advanced practice provider for CMM services to achieve defined outcomes through direct interactions with patients and providers in assigned care area(s). Functions as a mid-level provider to design, implement, and monitor therapeutic drug plans to achieve definite outcomes through direct interactions with patients and providers in assigned areas. Performs limited external physical assessment and performs medication reconciliation in alignment with established policies or procedures. Orders, performs, reviews, and analyzes appropriate laboratory tests and other diagnostic studies necessary to monitor and support the patient's drug therapy. Follows-up with patients on lab or test results to discuss the plan of therapy, i.e. changes in medication therapy, monitoring, and additional testing requirements. Determines if a referral to a physician is necessary when disease progression occurs, or adverse drug events require a higher level of treatment intervention Accepts urgent or same day patient visits including, but not limited to, patient medication review for polypharmacy, recent hospital or emergency department discharges, and co-managed care patients. Follows up via face-to-face, telehealth, eConsult, telephone, secure messaging, and other modalities to provide patient care Clinical - Indirect Patient Care Activities: Review patient medication regimens for clinical effectiveness, drug selection, dosing, contraindications, side effects, potential drug interactions, and therapeutic outcomes as required. Communicates findings with prescribers and provides appropriate alternatives to current treatment plans as needed. Obtains medication histories from patients and summarizes significant findings in the medical record and/or to the provider as appropriate. Monitor and report drug errors, adverse drug reactions, allergies, and patient compliance issues. Documents findings per facility procedures. Apply knowledge of normal laboratory values in the evaluation of patient care, recognizes significant abnormalities, and makes dose adjustment recommendations based on objective laboratory findings. Reviews and evaluates requests for non-formulary and restricted drugs for appropriateness and compliance with established criteria where applicable. Assists Pharmacy and Therapeutics committee with medication use evaluations and other safety initiatives. Documents clinical interventions in CPRS in a timely and professional manner as appropriate. Promotes and monitors compliance for established drug therapy policies. Works with providers to ensure compliance with national, VISN, and local initiatives. Program Management, and Administration: The incumbent has full line supervisory authority over clinical pharmacy specialists and clinical technicians located in Colorado Springs and Pueblo. The incumbent ensures that services provided meet or exceed professional and legal standards. Management efforts emphasize supervision of day-to-day details and clinical operations. Actively participates in ensuring that resources dedicated to clinical pharmacy activities are used effectively and required workload data is gathered and collected in an accurate and timely manner. Interviews employee candidates and recommends appointments, advancements, or promotions to the appropriate approving official(s). Manages affirmative action initiatives, grievances, EEO complaints and labor management issues while effectively supervising staff. Responsible for assigning duties; development of performance standards, performance evaluation, and completing ongoing professional practice evaluations; recommendations for appointment, awards, advancement, and, when appropriate, disciplinary action. The incumbent is required to cooperatively resolve routine and complex problems that affect other Pharmacy sections and/or other medical center services so that little or no intervention is needed for a higher-level supervisor. The incumbent defines and establishes effective process monitors and quality controls for the proper surveillance of clinical activities as outlined under direct and indirect clinical activities below. The incumbent plans short- and long-range objectives, achieves results, and ensures customer satisfaction to the extent possible within resource constraints. Works with providers to ensure compliance with national, VISN, and local initiatives. Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d). Education. (1) Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Today the sole degree is Doctor of Pharmacy.) (2) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL IBT). Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16. Grandfathering Provision. All licensed pharmacists employed in VHA in this occupation on the effective date of this qualification standard are considered to have met all qualification requirements for the title, series and grade held, including positive education and licensure/certification/registration that are part of the basic requirements of the occupation. For employees who do not meet all the basic requirements required in this standard, but who met the qualifications applicable to the position at the time they were appointed to it, the following provisions apply: Employees grandfathered into the GS-660 occupational series may be reassigned, promoted up to and including the full performance (journey) level, or changed to lower grade within the occupation, but may not be promoted beyond the journey level or placed in supervisory or managerial positions. Employees who are appointed on a temporary basis prior to the effective date of the qualification standard may not have their temporary appointment extended or be reappointed, on a temporary or permanent basis, until they fully meet the basic requirements of the standard. Employees initially grandfathered into this occupation, who subsequently obtain additional education and/or licensure/certification/registration, that meet all the basic requirements of this qualification standard must maintain the required credentials as a condition of employment in the occupation. If a licensed pharmacist who was retained under this provision leaves the occupation, the employee loses protected status and must meet the full VA qualification standard requirements in effect at the time of reentry to the occupation. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: GS-13 Pharmacy Supervisor. Responsible for the professional and administrative management of an assigned area in pharmacy service, to include budgetary execution. Such individuals have responsibilities for supervising multiple pharmacists and technicians. Manages people, maintains effective interdepartmental relations, and cooperates with other services to accomplish the medical facility's mission and goals. Develops performance standards, assures that the program area is compliant with all regulatory and accrediting body requirements, designs and implements orientation and training programs for staff, and develops and maintains systems to monitor the performance of staff activities. Develops policies and procedures relative to their assigned area. GS-13 Knowledge, Skills, and Abilities. Pharmacists assigned to this position must demonstrate the following KSAs: Ability to effectively supervise subordinate staff. Ability to communicate orally and in writing to persuade and influence clinical and management decisions. Expert understanding of regulatory and quality standards pertaining to pharmacy. Skill in managing people or programs. Preferred Experience: Highly desirable candidates will have recent experience in a current or previous job (including residency training) that includes direct patient care and comprehensive medication management responsibilities under an individual scope of practice; advanced pharmacy degree (PharmD); PGY1 Pharmacy Practice Residency; PGY2 specialty residency or equivalent experience in the specialty applicable to this position. Must possess math skills in order to convert medications to the correct dosing. The incumbent must be able to perform all activities of the position without restriction. This position involves lifting 25 pounds or less, able to visually confirm drug order correctness, and able to stand for periods of 30 minutes or greater.
