    Trinity Health of New England is looking for an experienced **Speech Language Pathologist** to join our mission-driven team in our **Outpatient Rehab Clinic in Middlebury.**

     

    The Speech Language Pathologist responsible for performing evaluations, direct patient treatment, and documentation for patients with mild-moderate cognitive deficit, neurological disorders, swallowing/oral motor impairment. SLP identifies appropriate goals and develops treatment plans to respond to those goals.

    What will you do:

    + Performs comprehensive evaluations.

    + Establishes functional goals based upon evaluation results targeting deficit areas, to help patients achieve a maximum level of functional independence.

    + Provides professional interaction with patients, family members, interdisciplinary team members, and other staff members and colleagues.,

    + completes all documentation in accordance with professional, organizational, and regulatory compliance standards.

    Minimum Qualifications:

    + Education: Graduate of an accredited program with a master’s degree in Speech-Language Pathology

    + State of Connecticut Speech-Language Pathology Licensure
