Description:

Master's level social worker (MSW) to provide care for PACE/LIFE participants at the Broad Street Center. Licensed social worker preferred.

Hours- Full Time

Position Purpose:

Join _Trinity Health PACE_ as a Social Worker to deliver compassionate and comprehensive social services to PACE participants and their families. You will plan, coordinate, and implement services that support physical, emotional, and social well-being, with a special focus on serving frail or elderly populations.

What You Will Do:

+ Conduct assessments and develop individualized care plans

+ Provide counseling, support, and education to participants and caregivers

+ Collaborate with the interdisciplinary team (IDT) on 24-hour care coordination

+ Facilitate resource referrals, financial eligibility, and advance care planning

+ Lead support groups, family meetings, and crisis interventions

+ Advocate for participants with external agencies and ensure quality care transitions

Minimum Qualifications:

+ Master’s degree in Social Work (MSW)

+ Two years of experience preferred, including one year with elderly/frail populations (or willingness to undergo training)

+ Experience in multidisciplinary teams and community-based care is a plus

+ Working knowledge of Medicare, Medicaid, and social service principles

+ Licensure in Social Work preferred
