    JOB SUMMARY

    This position is charged with strengthening the organized infrastructure for access to pharmaceuticals by uninsured and under-insured individuals. This role supports all CHC programs related to medication access. This includes evaluating most appropriate and efficient method to obtain pharmaceuticals based on multiple patient and organizational variables. Requests are accepted for medication access directly from CHC team members. The Coordinator is a member of the CHC Pharmacy and Therapeutics Committee. Among the key responsibilities is serving as a liaison between CHC, Inc. and its pharmacy partners. The Coordinator works directly with CHC’s Director of Clinical Pharmacy Services and nursing team in assuring that all CHC sites are in full compliance with Joint Commission standards for pharmacy, including compliance with CHC policies regarding storage and dispensing of pharmaceuticals. The Coordinator works with Access To Care (ATC), Center for Key Populations (CKP) and 340B teams to ensure appropriate medication coverage obtained.

    ROLE AND RESPONSIBILITIES

    Establish and maintain database for enrollment and tracking of patients in medication assistance programs. Exhibits and maintains in-depth knowledge of all aspects of diverse pharmaceutical company eligibility procedures and requirements and the 340B drug program. Utilize patient assistance programs from drug manufacturers, copay savings cards, the 340B program and other methods to remove barriers to treatment. Estimate that up to 50% of time will be spent in direct client service, assisting patients in applying for and receiving pharmaceuticals. Assist with and complete medication prior authorizations with assistance from clinical team when appropriate. Communicate with pharmacy partners to navigate processing issues ensuring prescribed therapy or alternative filled in timely manner and received by patient. Develops quarterly reports on status of CHC’s pharmacy access programs and submits to Director of Clinical Pharmacy Services. Provides outreach/educational services as requested. Maintains compliance with agency-mandated in-service training.

    QUALIFICATIONS

    + BA/BS degree in health, human service, or related field (preferred)

    + Clinical certification/background preferred (MA, LPN, CPhT)

    + 2+ years’ experience in community-based social or health services organization

    + High level of proficiency in Excel, Word, and other appropriate software

    + Valid CT driver’s license

    PHYSICAL REQUIREMENTS/WORK ENVIRONMENT

    Must be able to operate keyboard/PC, copier, telephone, voice mail and fax machine. Continuous sitting, some standing and walking necessary. Some driving required.

     

    40 hours per week.

    ADDITIONAL QUALIFICIATIONS

    Requires excellent oral and written communication skills for interaction with clients, professional contacts, and staff. Bilingual skills a plus; ability to use Language Line for translation service required.

    Confidentiality of business information is a requirement. Confidentiality must be maintained according to CHC policies

     

    Full access to patients’ medical records and encounter data. Particular attention to be paid to communication across agency/organizational lines. Works with Compliance Officer to ensure that all policies are adhered to.
