PERSONAL CARE ASSISTANT PRN

**Location:** Norristown, PA

Position Purpose:

This role impacts participants lives by providing essential personal care services and health support.

Position Details:

Role includes both home care and facility work. Home care assignments would be in the Norristown area with some assignments possibly in Pottstown.

Shifts for coverage would be Mon- Friday 8 am to 430 pm and weekends as needed 8 am to 430 pm.

Must be available to cover at least two weekdays and at least one weekend a month. Schedule is very flexible.

What you will do:

+ Conduct ongoing participant assessments and monitoring.

+ Provide assistance with daily activities, including toileting, grooming, and meals.

+ Aid in maintaining physical activity and exercise routines.

+ Identify safety hazards and respond to emergencies.

+ Assist with recreational activities, meal service, and housekeeping.

+ Report any changes in participant's condition to center staff.

+ Escort participants to appointments and assist with transportation.

Minimum Qualifications:

+ High School Diploma or equivalent.

+ Minimum of one (1) year of documented experience working with a frail or elderly population or if less than one (1) year of experience, must receive appropriate training from the PACE organization upon hiring.

+ Preferred experience in long-term care or geriatric programs.

+ Strong communication and organizational skills.

+ Valid driver's license, insurance and use of personal vehicle.

+ Ability to work independently and adapt to change.

+ Some travel within the region may be required.
