Job Title: Peer Coach Department: Behavioral Health Care Management Location: Rochester Mental Health Center Hours Per Week: 20 Schedule: Negotiable, 6 hour shifts. SUMMARY: To provide outreach, peer support and practical assistance to adults with severe and persistent mental illness who experience homelessness, or who are at risk of homelessness. To assist in the development of self-advocacy skills, and development of peer networks and formal resources to consumers who could benefit from ongoing support/mentoring in achieving personal goals. To provide continuity of peer support across the mental health service spectrum.
RESPONSIBILITIES:
+ Complete outreach visits to mental health patients while hospitalized, or waiting in acute care settings.
+ Visit area homeless shelters to provide outreach and peer support.
+ Consult with other relevant mental health services regarding outreach and peer support needs of consumers.
+ Provide consumers with basic, practical personal care items when needed.
+ Maintain contact with consumers as they transition within the mental health service system.
+ Assist consumers with basic needs (i.e. grocery shopping, obtaining medications, etc.) by accessing wrap-around funds associated with the program.
+ Assist consumers in accessing services or resources as needed.
+ Organize and maintain a resource list of services and contact persons/phone numbers.
+ Help the recipient access training as provided by the Mental Health Association, and other self-help organizations.
+ Assist recipients in establishing personal goals and priorities, and assert these to relevant service providers as needed.
+ Accompany consumers to community-based services to enhance linkage and support.
+ Help the recipient access training as provided by the Mental Health Association, and other self-help organizations.
+ Encourage linkage with vocational, educational, and/or social/leisure services.
+ Provide input regarding the needs of recipients within the structure of the PATH project.
+ Performs other duties as assigned.
REQUIRED QUALIFICATIONS:
+ High school diploma or equivalent plus additional specialized training.
+ Strong communication skills required.
+ Knowledge of community resources required.

S - Sedentary Work - Exerting up to 10 pounds of force occasionally Sedentary work involves sitting most of the time, but may involve walking or standing for brief periods of time. Jobs are sedentary if walking and standing are required only occasionally and all other sedentary criteria are met.

The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
