Overview
History of the Company: Superior Ambulance Service started in 1959 with one ambulance and today is the largest independent, locally owned, and operated emergency medical services provider in the Midwest. Superior employs more than 3,500 licensed EMTs, Paramedics and Nurses, operating a fleet of more than 800 ambulances throughout Illinois, Indiana, Ohio, Michigan, and Wisconsin. Superior also provides Critical Care, helicopter, and fixed wing emergency medical transportation.
Superior Ambulance Service is participating in a grant-based program amongst 80+ Emergency Departments Nationwide. The Community Paramedic-Led Transitions Intervention (CPTI) team will identify dementia patients to participate in a hospital to home transitions model of care. CPTI is a program designed to support patients discharged from the Emergency Department. The Paramedic coach will provide a post-hospital phone call(s) soon after discharge, one in-person home visit and up to 3 follow-up telephone calls over 30 days. During these encounters, the Paramedic coach will utilize learned motivational techniques, behavior modeling, skill transfer, and role play to improve the patient's self-management. Areas of focus include effective medication management, encouraging primary care follow-up, red flag awareness and maintenance of personal health goals. Paramedic coaches do not deliver traditional EMS associated services (e.g. they do not provide medical care or medical evaluation). The Paramedic coach will travel to the participant in a marked vehicle and in uniform.
Responsibilities
+ Perform the post-hospital phone call to develop the coaching relationship, explain the CPTI model, engage the person living with dementia (PLWD) and their care partner, and schedule the home visit.
+ Perform the home visit within ~5 days following discharge. Work with the individual or care partner (together= “dyad”) to identify their biggest health concern and their immediate goals for the next 30 days. Coach the individual through the four pillars of care.
+ Utilize active listening, motivation interviewing and teach back methods to develop individuals’ selfmanagement skills.
+ Assess the patients baseline activation level for self-care through use of a standardized assessment tool.
+ Complete three follow up phone calls to the individual on or about the 2nd, 3rd and 4th weeks following discharge.
+ Coaches will be responsible for completing accurate and timely charting of patient encounters in accordance with learned processes and be accountable for completion of the intervention in the appropriate time frame. Complete data entry after each encounter on CTI+ platform.
+ Participate in regular meetings with the program coordinator, CPTI national team, other CPTI coaches and various hospital partners for ongoing program management.
+ Establish effective communications and relationships with ED/hospital staff and CPTI partners.
+ Participate with quality assurance activities.
+ Follow program procedures for documenting and tracking.
+ Demonstrate strong organizational and time management skills.
+ Participation in all required didactic, clinical and lab education programs and maintenance of all required certifications and licenses.
+ Complete and maintain currency in all areas of safety education as required by Superior Ambulance, contracted air operators, and other healthcare partners.
+ Demonstrate effective communication skills with all external and internal customers, including.
+ patients and their families, hospital staff, the community, other Superior Ambulance staff and Critical.
+ Care administration. Represents the study and Superior Ambulance in a professional manner.
Qualifications
+ Valid Illinois Paramedic License
+ Valid Driver’s License
Training Expectations
+ The Paramedic Coach must complete a 40-hour provided CPTI training course. Upon completion, the candidate will receive a certificate in Community Paramedicine Associated CME hours.
+ Candidates must be available in August to complete the required online training.
+ Coaches must attend mandatory CPTI training (appx 4 weeks) and all applicable follow-up trainings prior to launch.
Physical Reequirments
+ Must successfully complete pre-employment testing including a physical and lifting agility examination.
+ Must be able to occasionally work outdoors in inclement weather.
+ Must be able to accommodate body positions and movements such as sitting, standing, twisting, crouching, bending, walking, climbing in and out of ambulance, climbing stairs, turning head and torso, bending at the arms, knees, wrists and fingers.
+ Must maintain good physical stamina, endurance, and body condition.
+ Must have full use of eyes, ears and full power of speech.
+ Must be able to handle stresses related to patient care in transport, without compromising personal or overall transport safety. All staff members are encouraged to make lifestyle choices that promote healthy living as it relates to job performance.
Schedule
+ Monday - Friday
+ Ability to work a flexible schedule. Days scheduled including start times may adjust weekly based on patient volume and needs.
+ Attend administrative meetings and other ED-LEAD personnel as needed.
Supervision
The Community Paramedic receives supervision from the Manager of Superior Mobile Health Services, the Executive Director of Critical Care Services, and the Program Medical Director.

Salary Range
USD $30.00 - USD $40.00 /Hr. rates offered based on years of experience and full time scheduled templates
