Job Description

Leads, plans and conducts nutritional programs to assist in the promotion of health and control of disease. Provides nutritional assessment, education and counseling to individuals and/or groups. Serves as a resource on nutrition and dietetics to nurses, health educators or other health care providers in the facility.

Job Responsibility

+ Leads, plans and conducts nutritional programs to assist in the promotion of health and control of disease.

+ Oversees the assessment of nutritional needs and provides nutritional counseling to assigned clients/patients.

+ Considers patients' cultural, educational, and learning issues.

+ Plans individualized nutrition education to patients, their families, and/or significant others based on their needs assessment.

+ Serves as a resource on nutrition and dietetics to nurses, health educators or other health care providers in the facility.

+ Ensures communication of patient information to referring providers and appropriate staff in a timely manner.

+ Ensures documentation of nutrition information discussed, written materials provided, and evaluation of patient's receptiveness and comprehension of the education provided.

+ Participates in professional meetings and conferences.

Job Qualification

+ Bachelor's Degree in Food, Nutrition or Dietetics, required.

+ Registered Dietitian, required.

+ Current Diabetes Educator Certification, required.

+ Minimum of five (5) years progressively responsible, related experience to ensure familiarity with scope of program.

OR

+ Bachelor's Degree in Nursing, required.

+ Current license to practice as a Registered Professional Nurse in New York State, required.

+ Current Diabetes Educator Certification, required.

+ Minimum of five (5) years progressively responsible, related experience to ensure familiarity with scope of program.

The salary range and/or hourly rate listed is a good faith determination of potential base compensation that may be offered to a successful applicant for this position at the time of this job advertisement and may be modified in the future. When determining a team member's base salary and/or rate, several factors may be considered as applicable (e.g., location, specialty, service line, years of relevant experience, education, credentials, negotiated contracts, budget and internal equity).

The salary range for this position is $70,470-$116,870/year
