    Position Summary

    Accountable for assisting consumers in the Involuntary Outpatient Commitment program including: ensuring the coordination of patient care, Psycho-education, Linkage to treatment, other services & entitlements. Works collaboratively with the Emergency Services staff , 2 Central, psychiatry, court system, and the State system of care. Supports the mission, values and philosophy of HMC.

     

    Primary Position Responsibilities

     

    + Accountable for the coordination of patient care for those consumers on the case load though out their involvement in theInvoluntary Outpatient Commitment Program.

    + Reviews records and maintains standards of HMC, JCAHO, DOH, NJ Division of Mental Health & Addiction Services.

    + Provides high quality Case Management and linkage to care.

    + Constructs treatment plans in conjunction with the coordinator and psychiatrist for the court and monitors consumer’scompliance.

    + Constructs Wellness and Recovery Action Plans and Psychiatric Advanced Directives with consumers.

    Qualifications

    Minimum Education:

    Required:

    High School diploma or equivalent

    Preferred:

    Bachelor’s Degree in Social Work, Psychology, or Equivalent

    Minimum Years of Experience (Amount, Type and Variation):

    Required:

    Personal experience of Mental Health Issues

    License, Registry or Certification:

    Required:

    Valid Driver's License.

    Knowledge, Skills and/or Abilities:

    Required:

    + Excellent interpersonal communication skills.

    + Ability to identify and resolve problems.

    + Computer literate, skilled in customer service.
