+ Responds to calls from triage and provides patient visits on request of designated triage nurse

+ Responds to medical or emotional crisis and/or death as requested by designated triage nurse

+ Provides skilled nursing care focusing on pain control and symptom management

+ Conducts hospice admissions as needed

+ Consults appropriately with attending physician and medical directors to coordinate patient care and receive orders

+ Assists RNCMs with their patient assignments by conducting scheduled after-hours visits as necessary to meet patient care needs.
