Position Purpose:

To provide Occupational Therapy services to patients to include evaluation, assessment, treatment program planning and implementation, and related documentation, and communication.

What you will Do:

+ Evaluates patients to obtain and interpret data necessary for treatment planning and implementation.

+ Develops treatment plans, directs, supervises and treats patients in accordance with treatment plan using treatment modalities.

+ Prepares written report of diagnostic findings, intervention techniques used, progress made and follow-up status in a time frame according to policy and procedure.

+ Consults and communicates with patient, family and other multi-disciplinary team members and contracted agencies.

+ Completes required documentation on patients and maintains other department records in accordance with departmental and health center policies and procedures.

+ Delegates and supervises activities of occupational therapy assistants and technicians, to assure efficient use of department resources.

Hours/Schedule:

.8 FTE (32 hours per week), day shift

Minimum Qualifications:

+ B.S., B.A., M.O.T., M.S., or M.A. degree required.

+ Licensed as an Occupational Therapist in the State of Iowa required.

+ Prefer demonstrated competence in an outpatient care setting for at least one year.

+ Must have excellent verbal, written communication and interpersonal skills.

+ Must provide proof of **Basic Life Support** ( **BLS Provider** -American Heart Association or Professional Rescuer-American Red Cross) prior to employment or within 30 days of hire and maintains current status.

+ Must provide proof of **Mandatory Reporting for Child and Dependent Adult Abuse** training prior to employment or within 6 months of hire and maintain current status per State of Iowa guidelines.
