    Summary: Assist homeless across the region access the centralized CoC regional list to obtain housing opportunities Work with the most vulnerable in the region to navigate resources, providers and housing. Work closely with shelter providers and frontline staff to give knowledge and understanding of the coordinated entry system.

    Primary Responsibilities:

    + Manage the CoC regional list. Keep it updated and current. Report housing placements.

    + Help shelter staff and outreach teams understand the coordinated entry system and how to help their homeless clients access it.

    + Work with the CoC “most vulnerable list to assist the homeless.

    + Oversee and hold monthly meetings with direct staff that do the work.

    + Work with and support the programs that also do housing work like the Housing Consumer Education Center, RTI, The youth program and the family and individual system.

    + Coordinate appropriate referrals from the regional list to make suitable placements.

    + Ensure that Case Managers complete assessments and stabilization plans in a timely and thorough manner.

    + Attend quarterly meetings with the CoC and other housing navigators around the state.

    + Attend SMOC’s managers meetings and triage meetings to share and learn more about the programs and individuals that are being served.

    + Manage and facilitate case conferences as needed to monitor and address ongoing or challenging issues.

    + Manage a comprehensive resource list to share with providers to help the clients they are working with.

    + Work on an extensive landlord list. Work on developing relationships with landlords around the region

    + Complete and document regular review of participant files and data entry.

    + Ensure the services are operated in compliance within all federal, state and local funding requirements, as well as, agency policies and procedures.

    + Be available to travel around the region to meet with providers, landlords and to attend meetings.

    + Provide on-call support as assigned. This may include being available to coordinate services in evening hours.

    + Engage all clients by understanding and addressing their needs whether within or outside the scope of work.

    + Attend & participate in engagement team meetings as requested and communicate effectively with clients and staff in other areas.

    + Maintain confidentiality of client, employee and agency information in accordance with federal and state laws and funder requirements.

    + Ensure compliance with program/department, agency and/or funder requirements, as well as, SMOC policies & procedures.

    + Other duties as assigned.

    Knowledge and Skill Requirements:

    + Bachelor's degree

    + Previous experience with homeless individuals

    + Housing search experience preferred.

    + Excellent organizational and interpersonal skills required.

    + Bi-lingual skills a plus .

    + Valid Driver’s license, reliable vehicle and evidence of insurability.

     

    Organizational Relationship: Directly reports to Director of Program Operations

     

    Physical Requirement: Ability to drive. Ability to ascend/descend stairs. Ability to lift up to 30 lbs

     

    Working Conditions: Travel within Massachusetts required . As part of the responsibilities of this position, the COC Housing Navigator will have direct or incidental contact with clients served by SMOC in various programs funded or administered through the Executive Office of Health and Human Services. A successful background check is required.

     

    Remote Work Option: Remote work is permissible in some positions at SMOC depending on the key functions and responsibilities. The CoC Navigator position is eligible to work from home 20% of the week in scheduling coordination with the department manager.

     

    Monday-Friday ; 9am-5pm
