Eddy SeniorCare has a rewarding opportunity for a RN Supervisor to lead our PACE Primary Care Clinic located at 385 Watervliet Shaker Road, Latham!
PACE is a unique program on the forefront of trends in healthcare, combining an insurance role (we are the Medicare/Medicaid insurer for our participants) with a comprehensive healthcare provider role (primary care, home health, adult daycare, transportation, all coordinated by an interdisciplinary team).
The focus of a PACE program is on the prevention of medical issues in order to allow the participants to safely stay living in the community.
The primary purpose of this position is to provide and supervise the day to day clinic nursing responsibilities of Eddy Senior Care at our Latham location. Within the PACE program, which includes a day center and rehabilitation gym, there is a fully-functioning medical clinic staffed by nursing and physicians. The participants enrolled in Eddy SeniorCare are seen semi-annually in the clinic for evaluations, and as needed when they are sick. The Physician in the PACE clinic averages 4-5 participants daily with 30-45 minutes for standard visits, and 60-90 minutes for assessments. The focus is not on the number of participants that can be seen in a day, but making sure that the team is addressing all of their medical needs.
The Nurse Supervisor would be responsible to oversee and work alongside clinic staff, as well as provide support to the provider team. They would assist in patient care, collection of speciments and the coordination of medication orders. They would assist in the coordination of urgent visits preparation for scheduled appointments. Above all, the Clinic Nurse Supervisor would work closely with the entire interdisciplinary team, including community nursing, therapy, social work, nutrition, transportation and activities.
Work schedule is generally M-F 8am to 4:30pm.
Pay Range: $34.75 - $53.60
Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
