Typical hours are 8:00 AM – 4:30 PM, Monday through Friday, with on-call and weekend rotations (approximately one weekend per month). Flexibility with accountability is key: you’ll plan your daily schedule based on your patient caseload.

Our Case Managers play a pivotal role in helping patients heal, thrive, and remain independent—while finding personal and professional fulfillment in the field.
Position Summary:
Responsible for the case management and provision of quality home nursing services to patients within the agency’s philosophy, policy, and Standards of Community Health Nursing Practice.
Education, License & Cert:
Graduation from an approved School of Nursing, Bachelor’s Degree in Nursing preferred.
Experience:
One‐year previous nursing experience with Medical/Surgical Nursing required. Prior Public Health/Community Health experience preferred.
Essential Functions:
1. Plans, implements, manages, and evaluates the provision of both professional and ancillary home health services (direct care and case management) to ensure that all patient’s needs are met and quality care provided in accordance with Federal, State, and program guidelines.
2. Is responsible for ensuring that he or she can meet the yearly competency requirements. Attends staff meetings and educational opportunities.
3. Must provide nursing care in the patient’s environment.
4. Manages the activities of the multi‐disciplinary team providing care to patient.
5. Demonstrates effective time management, flexibility and priority setting in patient care.
6. Must have excellent verbal and nonverbal communication skills and facilitate communication between team members and patients.
7. Independent judgement and excellent planning and problem‐solving skills are required.
8. Is responsible for LPN supervision.
9. Must have reliable transportation.
10. Must be able to accept on call assignments.

The pay rate for this position is $37.50 - $53.06 based on experience.
