What You’ll Do:

+ To provide support to staff and provide telephone interventions to maintain patients safely in their home, including scheduling home visits as needed.

+ Collaborate with Physician practices and hospital staff to influence effective management of patients in their homes.

+ Assess and identify the patient’s holistic health care needs.

+ Demonstrate competency in assessment and technical skills.

+ Possess and utilize knowledge of pain control & symptom management.

What You’ll Bring:

+ Current R.N. license in the State of Maine.

+ ANA certification in Home Health or Community Health Nursing is encouraged.

+ Current CPR card required.

+ One year of recent clinical nursing experience is required.

+ A reliable car with insurance.
