    Work Schedule: Full time, Monday through Friday 8 am to 4 pm (40 hours), required to travel to Orleans County and Niagara County twice a month.

     

    A Day in the Life of a Treatment Coordinator:

     

    As a full-time Treatment Coordinator, you will play a vital role in our clinic team, ensuring that services are timely and comply with all regulations. Your expertise in coordinating clinic services will directly contribute to enhancing patient experiences and overall team effectiveness.

     

    + Review referral packets to ensure that all requirements are present and verifies insurances.

    + Serves as primary liaison throughout the treatment between referral source, individual, treatment providers and collaterals to promote communication and integration of program services

    + Oversees records ensuring completion of all necessary information in accordance with policies, regulations and insurance requirements

    + Delivers group presentations for families and other service providers regarding available Clinic services and supports

    + Reviews discharge reports ensuring compliance and appropriate communication relating to follow up and/or recommendations

    + Tracks annual authorizations and coordinates reviews required by Medical Director

    + Maintains a high level of confidentiality at all times

    Who We Are Looking For:

    + Bachelor's Degree or Registered Nurse

    + Must possess a current, valid New York State driver's license that meets the Agency driver standards
