Work Schedule: Full time, Monday through Friday 8 am to 4 pm (40 hours), with flexibility to meet program needs

A Day in the Life of a Care Manager:

As a dedicated full-time Care Manager, you will play a pivotal role in providing exceptional and comprehensive care management services to adults. You will be responsible for maintaining contact with members throughout Wayne, Monroe, Ontario and Seneca counties.

+ Provides assistance with housing, transportation, food, clothing and other barriers.

+ Provides self-management support, health promotion, connection/referrals to providers, community based organizations, social supports, transitions of care and crisis support with the goal of decreasing barriers to attending appointments and following the plan of care

+ Provides coordination with Primary/Specialty medical care, acute and outpatient medical, mental health, substance abuse services, and other care managers involved in supporting the client

+ Provides crisis intervention, addressing events such as emergency department visits or inpatient or other crisis events to assure interventions are effective and necessary modifications to plan of care are made

+ Home and field visits to provide counseling, crisis intervention referral and advocacy services, coordination of services and assistance with basic needs

Who We Are Looking For:

+ Bachelor's Degree in human services or related field or New York State License Registered Nurse

+ Two years' relevant experience providing direct services to person diagnosed with mental disabilities, developmental disabilities, alcoholism or substance abuse

+ Computer experience with working knowledge of Microsoft Office programs including but not limited to Excel, Outlook and Word.

+ Must possess a current, valid New York State driver's license that meets the Agency driver standards
