The Pediatric Stem Cell Transplant Department is a highly specialized, multidisciplinary program serving patients with a range of complex conditions, including high-risk cancers, bone marrow failure syndromes, immune deficiencies, and rare genetic disorders. They provide seamless support through all phases of care, from pre-transplant evaluation through inpatient treatment, and into long-term survivorship and late effects monitoring. Patient Navigators are essential members of the team, guiding families through the medical, emotional, and logistical aspects of a complex care journey. They act as advocates and liaisons, ensuring timely communication, helping to remove barriers to care, and connecting families with resources tailored to their unique needs.
Key Responsibilities:
+ Meets with or contacts patients and families to review care plans, explain available hospital and community resources, and provide supporting documentation; refers patients to appropriate clinic or specialized services.
+ Assesses patients for resource needs and acts as a liaison with both internal and external providers to ensure access to services, including follow-ups with hospital and community agencies.
+ Assists with scheduling multidisciplinary appointments, reviews appointment instructions, and coordinates logistics to support adherence, including reminders and transportation.
+ Helps patients apply for insurance coverage and communicates insurance details to care teams to confirm service eligibility.
+ Builds and maintains relationships with community resource providers and internal staff; conducts outreach and reports referral outcomes to appropriate sources.
+ Documents patient interactions and care, tracks relevant data, supports strategic planning, and participates in care planning meetings to facilitate discharge or ongoing care.
Minimum Qualifications
Education:
+ High School Diploma/GED Required
+ Bachelors degree in Human Services or other related field preferred
Experience:
+ A minimum of one of closely-related experience
+ Experience with the clinic population or work with community resources is preferred.
