    Home Health Physical Therapist – Centralia, WA (Full-Time)

    Job Description:

    Contract Type: Flexible (26 weeks to 1 year, orTemp-to-Perm)

     

    Schedule: Full-Time, 40 hours/week

     

    Local Pay $70/hr

    Travel Pay Package:

    + Hourly Rate: $45/hour (40 hours/week)

    + Meals Stipend (Non-Taxed): $476/week

    + Housing Stipend (Non-Taxed): $770/week

    + Total Gross Weekly: $3,046

    Position Summary:

    We are seeking a dedicated and experienced HomeHealth Physical Therapist to join our team in Centralia, WA. This roleis ideal for a compassionate clinician who thrives in a flexible,patient-centered environment. You will provide in-home therapy services topatients recovering from illness, injury, or surgery, helping them regainmobility and independence.

    Key Responsibilities:

    + Deliver skilled physical therapy services in patients’ homes

    + Develop and implement individualized care plans

    + Collaborate with interdisciplinary teams to ensure quality care

    + Accurately document visits and patient progress using Homecare Homebase (preferred)

    + Meet a productivity expectation of 26 points per week

    Qualifications:

    + Licensed Physical Therapist in the state of Washington (required)

    + Minimum 1 year of experience in physical therapy (required)

    + Experience with OASIS documentation and Homecare Homebase (preferred)

    + Valid Driver’s License and reliable transportation

    + Must meet onboarding requirements (see below)

    Onboarding Requirements:

    + Background Check

    + Motor Vehicle Record Check

    + Proof of Auto Insurance

    + WA PT License Verification

    + BLS Certification

    + Driver’s License

    + TB Test

    + 9-Panel Urine Drug Screen

    + Hepatitis B Vaccination

    + Physical Exam
