The long-term care social worker will work with residents in the nursing home by identifying their psychosocial, mental, and emotional needs along with providing, developing, and/or aiding in the access of services to meet those needs. The social worker is responsible for fostering a climate, policies, and routines that enable residents to maximize their individuality, independence, and dignity. This climate shall provide residents with the highest practical level of physical, mental, and psychosocial well-being and quality of life.
Supervisory Responsibilities:
+ May supervise social work students.
Duties/Responsibilities:
+ Works closely with members of the health care team in the development of the plan of treatment, including discharge plans, based on the psychosocial assessment and accepted social work interventions.
+ Develops working relationship with leaders in community resource groups, representing the agency at meetings and on various committees that address the social needs of patients such as Council on Aging, Developmental Services, etc.
+ Maintains knowledge of the field including Medicare, Medicaid, and other third-party reimbursement sources.
+ Participates in planning and implementing quality improvement activities.
+ Provides positive customer relations in dealing with residents, families, physicians, fellow department employees, other hospital departments, referral resources, service agencies, and others.
+ Provides education to residents/families regarding Advance Care Planning and assists with completing advance directives if the resident desires
+ Complete required documentation for all resident/family encounters and keep records up to date according to policy
+ Assist with financial concerns that have been identified by the resident/family
+ Attend and actively participate in meetings and committees that are assigned
+ Reports to proper legal authorities if you have reason to believe a patient has been physically, emotionally, or sexually abused or a been a victim of theft or misappropriation of property. Consult with staff with neglect or abuse concerns that are identified
+ Attend and participate in community education programs to stay professionally informed
+ Provide training to staff in form of "in-services" and individual counseling regarding social service-related areas
+ Create and maintain an atmosphere of warmth, cheerfulness, enthusiasm and giving quality of service you would want to receive personally. Reflect the core values of Glendive Medical Center
+ Assists as the backup person for acute care social services when he/she is out of the office
+ Respect all resident rights, including confidentiality of patient information
+ Assists with the Admission/Referral Process for prospective residents for long term care. Maintains the facility waiting list.
+ Assist residents in obtaining adaptive equipment, clothing needs, or personal items as needed. Assist with referrals and obtaining services outside the facility (i.e., talking books, absentee ballots, community transportation van)
+ Helps build relationships between residents and staff and to teach staff how to understand and support resident's individual needs.
+ Promotes actions by staff that maintain or enhance each resident's dignity in full recognition of each resident individuality.
+ Spiritual needs of the resident will be referred to the minister/priest of choice with consent of resident.
+ Discharge planning services for resident who would like to return to the community
+ Identify ways to meet the needs of a grieving resident
+ Adheres to department/hospital policies.
Required Skills/Abilities:
+ Physical, sensory, and cognitive abilities sufficient to perform essential functions.
+ Good interpersonal and written and verbal communication skills. Attention to details.
+ Flexible and adaptive to new and difficult situations; able to adapt to fast-paced environment
+ Active listening skills
+ Able to multitask and have good organizational skills
+ Flexible and willing to learn and/or assist with implementing new ideas and processes
+ Great customer interaction and rapport
+ Time management skills
+ Self-motivated with the ability to accomplish multiple tasks within tight deadlines but also a effective team member for the department/organization.
+ Proficient with the computer and software programs such as Microsoft Office
Education and Experience:
+ Bachelor's degree in social work or in a Human Service Field but not limited to, sociology, gerontology, special education, rehabilitation counseling, and psychology.
+ One year of supervised social work experience in a healthcare setting working directly with individuals.
Physical Requirements:
+ Prolonged periods of sitting at a desk and working on a computer and standing, bending, and moving.
+ Must be able to lift up to 15 pounds at times.
+ May be exposed to patients with infectious diseases
+ Occasionally may be exposed to an aggressive agitated customer
+ Must have good mobility to get around facility safety and quickly
+ Must have good hearing and speech ability to communicate with patients and families
Additional Job Information:
+ Pay: This is a non-exempt, hourly position and employees are paid on a bi-weekly basis
+ Bonus: Position may be eligible for recruitment incentives.
+ IE: Sign-on, Retention, Moving Assistance, Tuition Reimbursement, etc.
+ Typical Schedule: Monday - Friday (8AM - 5PM)
+ Location: On Site - 202 Prospect Dr, Glendive MT 59330
+ Travel: 50/50 covering 4 counties
