    Baystate Health is hiring a **Hospital Case Manager (RN)** to help patients navigate their care with compassion, efficiency, and clinical expertise.     Position Highlights:

    + Collaborate with patients, families, doctors, and care teams to develop and manage patient-centered plans

    + Assess needs, coordinate discharge planning, and ensure safe transitions of care

    + Use clinical criteria to review admissions and ongoing care for medical necessity

    + Communicate with insurance companies to support timely, appropriate reimbursement

    + Participate in daily unit rounds to guide care progression

    Requirements:

    + Registered Nurse (RN) with an active license

    + **Bachelor’s Degree preferred** (Associate’s accepted with approval)

    + **3–5 years** of recent acute care (med-surg) nursing experience

    + **1–3 years** of hospital case management experience (preferred)

    + Strong communication, critical thinking, and teamwork skills
