    Description

    Case managers screen adults, children and families for insurance and other benefit program eligibility. Case managers complete applications for eligible families and provide intensive tracking and follow-up with applicants to ensure that benefits are successfully accessed and all required supporting documentation is obtained.

    Qualifications

    + Qualifications

    + Works collaboratively with physicians and other health care personnel in patient evaluations and treatment to further their understanding of significant social and environmental factors underlying patients' health problems.

    + Provides direct case management services to assist patients in receiving necessary resources and services such as: Medicaid, Child Health Plus, Welfare, DAS, Social Security, Child Care, Home Care, Housing/Shelter/Safe Homes, Substance Abuse Treatment, Mental Health Treatment, HIV Services, Legal, other Health Insurance etc.

    + Completes sliding scale, 340 B pharmacy benefits, financial waiver, and cancer service applications as needed.

    + Helps assist patients with insurance options including pooled trusts.

    + Documents all patient services in patients electronic medical record, and closes encounters within 24 hours.

    + Prepares case management assessments, patient histories, service plan and reports.

     

    Advocates when necessary for patients with outside systems.
