    Locations:

    Lebanon, NH with required travel to system member locations throughout Vermont and New Hampshire, as needed.

    The Clinical Nurse Liaisonis responsible for facilitating referrals to Visiting Nurse and Hospice for VT and NH. The Clinical Nurse Liaison will provide support and education through collaboration with patients and family members, referral sources, providers and other agency departments, and also manage the referral process of patients to facilitate timely initiation of services. This unique position will work primarily from Dartmouth Health Medical Center and will travel to various DH system member sites, such as Alice Peck Day Memorial Hospital, Valley Regional Hospital, Mount Ascutney Hospital, etc., as needed to provide referral support and education pertaining to services offered by Visiting Nurse and Hospice for VT and NH.

    The Clinical Nurse Liaison:

    * Reviews provider orders, medical records, and related materials to determine patient clinical needs; uses clinical judgment to determine patient eligibility for services.

    * Educates referring providers, care managers, discharge planners, patients, and families on the scope of services offered by VNH programs.

    * Performs informational visits to prospective patients and families.

    * Continues professional development by participating in continuing education programs, conferences, and workshops.

    * Builds the brand of VNH as a high-quality provider of services.

    Minimum Qualifications:

    * Current or eligible to apply for RN license in both VT and NHora compact license required.

    * Valid driver’s license and reliable transportation is required.

    * Demonstrated knowledge of current home health and hospice clinical standards of practice required

    * Home health or ambulatory experience preferred.

    * Knowledge of Vermont & New Hampshire State regulations, accreditation-based standards as applicable, and Medicare Conditions of Participation preferred.
