    *Description*

    Job Summary:

    The Office-Based Cardiac Surgery APP is a critical member of the cardiac surgery team, providing comprehensive outpatient care to surgical patients. This role includes preoperative evaluations, patient education, postoperative office visits, coordination of the Aortic Aneurysm Surveillance Program, and leadership in hospital readmission reduction efforts. The APP will work closely with the surgical team while enjoying a 4-day workweek, focusing on delivering exceptional patient-centered care from preoperative preparation through postoperative recovery.

    Primary Responsibilities:

    * Conduct detailed preoperative assessments, including thorough medical histories, physical examinations, pre-operative testing and risk evaluations, complete pre-operative notes and assessments

    * Provide preoperative patient education, including:

    * Explaining the surgical procedure, hospital course expectations, postoperative care, and recovery timelines.

    * Reviewing preoperative instructions regarding medications, fasting, and activity restrictions.

    * Addressing patient and family questions and concerns to ensure informed consent and readiness for surgery.

    * Perform postoperative office visits, including:

    * Evaluating wound healing, removing sutures or staples, and managing common postoperative issues.

    * Reviewing imaging, lab work, and adjusting postoperative medications as needed.

    * Counseling patients on activity progression, cardiac rehabilitation, nutrition, and lifestyle modifications.

    * Participate in pre-discharge education while patients are still hospitalized, ensuring a smooth transition to outpatient care by:

    * Reinforcing discharge instructions on wound care, medications, signs of complications, and follow-up plans.

    * Educating patients and families about symptom monitoring and when to seek medical attention.

    * Organize the Aortic Aneurysm Surveillance Program, including:

    * Scheduling and reviewing imaging studies (CT scans, echocardiograms).

    * Monitoring patients with aortic pathology and coordinating timely surgical interventions.

    * Providing education on aneurysm progression, surveillance intervals, and risk factors.

    * Actively reduce hospital readmissions by:

    * Rapid outpatient follow-up for high-risk discharges.

    * Early recognition and management of post-surgical complications in the office setting.

    * Collaborating with home health services, case managers, and other outpatient providers.

    * Triage and manage patient concerns via phone calls, secure messages, or same-day office evaluations.

    * Assist with minor outpatient procedures, such as wound care, drain management, and anticoagulation monitoring.

    * Accurately document all patient interactions, assessments, and management plans in the electronic medical record (EMR).

    * Contribute to quality improvement initiatives, patient education materials, and multidisciplinary team efforts.

    Schedule:

    * Four 10-hour days per week (flexible scheduling available).

    * Minimal to no weekends, nights, or on-call responsibilities.

    Qualifications:

    * Graduate of an accredited Physician Assistant or Nurse Practitioner program.

    * Certified by the National Commission on Certification of Physician Assistants (NCCPA).

    * Active or eligible PA or NP license in NYS.

    * BLS and ACLS certifications required.

    * Experience in cardiac surgery, cardiology, vascular surgery, or thoracic surgery preferred.

    * Excellent clinical assessment, patient education, and critical thinking skills.

    * Strong organizational and communication skills, with a focus on continuity of care.

    * Ability to work independently and as part of a multidisciplinary team.

    * Proficiency with EMR systems and outpatient workflows.

    Work Environment:

    * Primarily outpatient cardiac surgery clinics with occasional collaboration across hospital-based services for pre-discharge teaching and transitional care planning.

    Physical Requirements:

    * Ability to stand, walk, and sit for extended periods.

    * Ability to occasionally lift or assist with patients or clinical equipment.

    * Manual dexterity for outpatient clinical procedures.

    Salary Range:

    Salary Range: $145,000- $173,000. yearly
