Patient Navigator
The University of Miami/UHealth department of Neurology has an exciting opportunity for a Full Time Patient Navigator to work at the UHealth Medical Campus. The Patient Navigator (H) facilitates patients’ ability to obtain services by identifying barriers to care, coordinating the development of treatment plans, and monitoring the implementation and follow-up of services ordered by caregivers.
Core Functions
+ Conducts patient evaluations to obtain information necessary for the development of an effective treatment plan.
+ Ensures patients have access to insurance and social service programs for which they are eligible.
+ Provides patient reminders of upcoming appointments and acts as liaison between patients and providers.
+ Identifies and resolves real or perceived barriers to care.
+ Educates and counsels patients on issues related to their health and life situations.
+ Maintains current knowledge of community health, welfare resources, and any other programs available to patients.
Department Specific Functions
+ Collect, review, and analyze complex and/or confidential data to prepare reports, charts, budgets, and other presentation materials.
+ Collect records and assist with appointments for referrals from providers for New Patient Visits.
+ Review physician schedules in advance and to ensure all necessary tests have been completed or ordered prior to visits, including reviewing charts of New Patient Visits; ensure tests are ordered before the visit and coordinate with the nurse or provider.
+ Make patient follow-up calls and no-show tracking callbacks; serve as the liaison between patient and provider.
+ Responsible for all pre- and post-visit care coordination, including reviewing patients' After Visit Summary (AVS) for post-visit instructions and checkout orders for testing by coordinating program activities with other programs, departments, or staff to ensure optimum efficiency.
+ Assist in the management of UChart messages.
+ Develop and enhance processes aimed at improving the clinical experience for referred patients or physicians.
+ Help patients and their families obtain referrals to specific specialists.
+ Follow up with referring providers to ensure care has been provided by the specialist.
+ Make suggestions to leadership on process changes to improve overall program performance by evaluating program effectiveness, analyzing results, and recommending appropriate actions.
+ Collect patient parking tickets to validate them prior to the end of the patient visit, serving as the point personnel for any patient-related satisfaction concerns in and out of clinics.
+ Navigate patients through their visits to ensure they make it to all needed appointments and referrals.
+ Ensure patients have all testing completed before they return to the physician for a follow-up.
+ Serve as the point person for patient sooner appointment requests for the Division of Movement Disorders.
+ Help resolve patient concerns and complaints, performing service recovery.
+ Assist other Patient Navigator staff with their workload and provide support as needed.
+ Provide in-person, hands-on support to patients during clinic days, including greeting, check-in assistance, escorting to exam rooms, and addressing real-time patient needs.
+ On-site presence is required on clinic days, while other responsibilities may be performed remotely on non-clinic days, as appropriate.

CORE QUALIFICATIONS
+ High School diploma or equivalent
+ Minimum 1 year of relevant work experience

+ Ability to handle difficult and stressful situations with professional composure.
+ Ability to maintain effective interpersonal relationships.
