Key/Essential Functions & Responsibilities
Job Coach:
+ Manages a caseload of individuals with I/DD seeking employment opportunities and performs pre-placement, intensive job coaching and long-term follow-along tasks for assigned individuals.
+ Obtains referrals, completes intakes, and enrolls new clients.
+ Completes assessments of participant’s skills, strengths, interests, support needs, etc. to determine job opportunities consistent with their interests, abilities and identified work goals.
+ Works one-on-one and/or in small group setting with participants to develop job seeking and vocational skills and to assist in obtaining and maintaining employment.
+ Develops career plan with participant, consistent with Individual Service Plan (ISP) and employment goals and assists in setting attainable employment goals.
+ Assists participants with resume building, interview skills, applications, and other job seeking skills.
+ Provides vocational training and develops and/or reviews soft skills and professional behaviors necessary to support employability.
+ Supports job development, including, but not limited to conducting outreach to form relationships with local businesses and targeting local jobs, linking the job seeker with customized employment opportunities, meeting with employers to identify support needs, etc.
+ Conducts job searches for or with participant, focused on competitive employment consistent with individual goals, skills and interests.
+ Regularly travels across assigned region(s) to visit employed individuals at their workplaces, providing on-site job coaching and direct training, travel training, assisting with orientation and new hire activities and supporting communications with employer.
+ Provides ongoing support to maintain successful employment including follow-up contact and/or visits, addressing issues/concerns and changes in work duties, assisting in career planning, etc.
+ Reviews and completes all required paperwork and documentation to ensure compliance and/or billing, as applicable, completely and accurately and within appropriate timeframes.
+ Remains up to date with mandatory and other trainings.
+ Maintains communication with team on individual progress and needs and escalates issues and concerns to supervisor for support, as needed.
Group Facilitator:
+ Leads and assists in facilitation of pre-vocational group sessions, consistent with DDD regulations and NIPD-NJ program model.
+ Provides individual assistance, within group setting, when needed.
+ Ensures timely and accurate documentation of services in compliance with DDD regulations and in appropriate format and electronic systems for all documentation related to group and caseload, including billing details.
+ Works with program leadership to identify, develop and/or update and enhance program curriculum according to the latest evidence-based practice, incorporating new teaching methods and content.
+ Conducts goal-setting activities with participants to support development of person-centered goals, and regularly monitors and evaluates each participant’s progress through observations and using quarterly assessments.
+ Ensures participants engage in meaningful activities that support progress towards their Individualized Service Plan (ISP) and program-specific goals and assists in redirecting strategies or activities to support goal achievement.
+ Assists with participant intake interviews to assess individuals’ acuity, interests and motivation/ability to participate in group setting and placement, as needed.
+ May contact family and/or Interdisciplinary Team (IDT) regarding progress, plans and/or any issues or concerns that arise, if needed.
+ May conduct program outreach, including presenting information sessions at schools and other agencies and attend transition fairs, conferences and other events to recruit program participants and market pre-vocational and vocational services.
+ Performs all other duties, as assigned.
Minimum Qualification Requirements including education, experience, licensure/certification, etc. and essential physical capabilities (e.g. lifting, assisting lifting, standing, etc.)
+ Bachelor’s degree in psychology, social work, counseling or related field; or
+ A satisfactory equivalent combination of education, training and/or experience, however, all candidates must have a Bachelor’s degree; and
+ Be familiar with the demands and expectations of business and industry; and
+ Must be at least (20) years of age.
+ New Jersey State drug test screening is mandatory prior to employment and randomly/for cause, as applicable
+ Must hold a valid Driver’s License with no more than 5 points
+ Ability to travel regularly across Northern NJ to support participants
+ Excellent interpersonal and communication with team members, supervisor, and people we support
+ Excellent time management and written communication skills
+ Able to work independently
+ Enthusiastic and friendly
+ Strong problem-solving skills
+ Proficient in Outlook, Microsoft Word, PowerPoint, and Excel
Preferred Qualification Requirements (desired requirements beyond MQRs above)
+ Master’s degree in Psychology, Social Work, Counseling or related field
+ Prior experience providing group/individual counseling and/or vocational services to individuals with I/DD strongly preferred
Schedule: Parttime
Compensation: $26.73
