Description:

Position Purpose:

+ Health Information Specialist II performs a variety of activities in the HIM Department by providing customer service, greeting patients and visitors on the phone or in person; reviewing and processing requests for release of information to patients and for continuity of care; and analyzing medical records for deficiency and quality Specialty: Hospice

+ Location: Mount Carmel St.Anns: 500 S Cleveland Ave, Westerville, OH 43081

+ Hours of office: 40 hour work week

What You Will Do:

+ Greets all customers in person and on phone and assists with their requests for health information.

+ Screens all ROI requests (in person, via mail, fax, email and telephone) prior to processing. Ensures authorization, release, and verification processes are followed.

+ Reviews, processes, and tracks requests for Protected Health Information. Sorts mail and distributes request to appropriate location or team.

+ Maintains all necessary records regarding the disclosures and tracking of requested information.

Minimum Qualifications:

+ Education: High School graduate is required. Associate Degree in Health Information Management preferred.

+ Licensure / Certification: Certification as a Registered Health Information Administrator or Registered Health Information Technician preferred.
