Salary: $1090 / Week

Essential Duties and Responsibilities:

+ Provides necessary counseling as required for the patient/client

+ Assesses the patient/clients’ psychosocial needs, and evaluate the progress with the care team and/or supervisor

+ Collaborates with all disciplines to plan and evaluate goals for each patient/client

+ Focuses on assisting patients/clients through the discharge process of both in and out-patient treatment facilities as applicable

+ Utilizes a variety of therapy techniques and treatment plans

+ Facilitates patient progress and referrals with other agencies

+ Adheres to all Amergis and Facility policies and procedures

+ Observes and reports patient/client progress per facility protocol

+ Performs other duties as assigned/necessary

Minimum Requirements:

+ Licensing/Certification according to state/facility/contract requirements

+ The Counselor may possess an Associate’s Degree, Bachelor’s Degree or Master’s Degree in Psychology, Sociology, Social Work, Counseling, Nursing or other Human Development Major

+ Complies with all relevant professional standards of practice

+ Participation and completion of Amergis' Competency program when applicable

+ Current CPR if applicable

+ TB questionnaire, PPD or chest x-ray if applicable

+ Current Health certificate (per contract or state regulation)

+ Must meet all federal, state and local requirements

+ Successful completion of new hire training as applicable to job site

+ Understand patient confidentiality and HIPAA requirements

+ Ability to effectively elicit/provide information to and from appropriate individuals (including, but not limited to, supervisors, co-workers, clients) via strong communication skills; proficiency in the English language is required

+ Computer proficiency required

+ Must be at least 18 years of age
