    About the work unit

     

    The Division of Regional Centers (DRC) is responsible for the planning, coordination, monitoring, and execution of a multi-million dollar, federal, and state-funded service delivery system for people with intellectual and developmental disabilities, policy management, regulatory compliance, and legislative initiatives.

     

    The Community Services (CS) team is responsible for Admissions, Consultations, and Transitions into and out of the three Regional Centers across the State of Colorado. This includes (but is not limited to) being the first to review referral information for admissions, communicating/coordinating with outside agencies (Case Management Agencies/CMAs, Service Agencies, hospitals, jails, etc.), completing relevant consent and other admission/transition forms, ensuring services are in place and transitions are thorough and timely. If an individual is accepted for admission to the Regional Centers, this position is the liaison between community providers and the regional center. This work unit also helps to develop policies and procedures related to admissions and transitions that support both Intermediate Care Facilities and Home and Community Based Waiver services.

     

    Provide statewide Community Support Team (CST) services. This work unit is responsible for reviewing all referrals for Community Support, assigning an appropriate consultation team, and coordinating and attending all CST meetings. As the lead on CST cases, this unit provides expertise on services and support in the community, helps to tap into resources that have not been accessed, and can assist community teams when barriers exist.

     

    As part of being a TCA, this unit also takes the lead on formal transition services, through transition planning with the Interdisciplinary Teams and by joining transition teams when an individual leaves the regional center for community placement. Transition follow-up can last as long as support is needed and the individual has what they need in their new community placement.

     

    This work unit exists to safeguard and advocate for the human/civil/legal rights of the individuals residing at any one of the three state Regional Centers. The range of Social Services includes (but is not limited to) assessment of needs, oversight of admission/transition planning, development of treatment plans with the Interdisciplinary Teams (IDT) including developing appropriate transition criteria, liaison with mental health agencies, CMAs, and other outside entities, and advocacy for individuals and their rights.

     

    Outreach and Training. This work unit develops and conducts training on Community Support Team services and Regional Center processes to Regional Center staff and external entities such as CMAs, Services Agencies, hospitals, law enforcement agencies, and legal entities. This unit also holds annual Community Provider Fairs and other events

    Description of Job

    About the Position:

    This position assists with Community and Transition Support Teams and provides consultation for admissions, consultation, transitions, and outreach/training for all three regional centers. Additionally, as noted above, this position will provide Transition Coordination as part of being a TCA.

    Primary Job Duties:

    Admission and Community Support Team (CST)

    + In cooperation with the supervisor, assists with Admission and CST coordination at the regional centers.

    + Review referral information in consultation with the Community Support Team and Placement Manager.

    + Exchanges information with CMAs and other involved agencies including service agencies, advocacy agencies, attorneys, state hospitals, parents, guardians, and other team members.

    + In collaboration with the supervisor, develops, implements, and trains policies and procedures related to admissions and CST.

    + Helps with data tracking related to admissions and CST.

    + In cooperation with the supervisor, reviews documentation for Admissions and CST services.

    + In cooperation with the supervisor, determines appropriate documents on Medicaid verification, and financial documentation, and reviews appropriate consent forms.

    + Contributor in revising policies related to Admission and CST for the three Regional Centers.

    + Assists with SIS level recommendations for community teams, including providing information about community safety risk, Support Level Review (SLR), or re-evaluation.

    + After being assigned as CST Lead for referrals by supervisor, assigns additional consultation team members based on referral information and individual needs.

    + Provides guidance and direction on CST meetings and recommendations.

    + Provides expertise and consultation on multiple services and supports in the community.

    + Assists teams when barriers exist.

    + Decides when cases should be escalated to the Community Support Team and Placement Manager.

    + Coordinates completion of all admission paperwork including consent for various services.

    + Ensures that financial paperwork is completed and gathers prior financial information such as bank statements and other forms.

    + Gathers all pertinent documentation prior to admission such as medication lists, birth certificates, Medicaid cards, etc.

    + In collaboration with the supervisor, advises, counsels, and informs referring agencies on the types of services provided by regional centers.

    + Coordinates and attends pre-admission meetings for individuals coming into the regional center, which includes ensuring transition criteria are discussed and addressed.

    + Thorough follow-up with community teams following CST meetings including contact with parents, family members, guardians, service providers, CMAs, and other team members.

    Transition Coordination Agency (TCA) and Transition Support Team (TST) Lead

    + Supports the Community Support Team and Placement Manager in maintaining sufficient documentation of all transition coordination activities performed.

    + Coordinates referrals and visits to meet rule and regulation requirements.

    + Assists in developing, establishing, and maintaining procedures sufficient to execute the TCA according to regulations.

    + This position coordinates all transitions and ensures that all the necessary components have been completed to move forward with the transition.

    + Organize data systems related to transitions and TST following any legislative guidelines.

    + Ensures that all documentation is accurate and communicates service and support needs to the accepting CMA and/or Program Approved Service Agency (PASA).

    + Consults with their supervisor regarding barriers to transition.

    + Responsible for transition planning, which includes training, support, follow-up, and connection with community services.

    + Consults and collaborates with single entry points, RAE’s, and other community agencies for resource development.

    + In cooperation with the supervisor, develops processes and documents in collaboration with stakeholders.

    + Acts as lead for TST services, including attending meetings and completing all notes and action plans.

    + Provides input on services and supports in the community.

    + Participates in assigned Interdisciplinary Team meetings to include transition planning and leading the TST, which includes training, support, follow-up, and connection with community services.

    + Utilizing Therap for billing purposes.

    + Staying up to date with training through the HCPF portal.

    Outreach and Training

    + In collaboration with the supervisor, determines and develops training relevant to the needs of the regional center and the community.

    + Helps to conduct new and ongoing training with outside entities such as hospitals, law enforcement agencies, CMAs, service agencies, etc.

    + Trains regional center staff.

    + Plans, organizes, and holds the annual Community Provider Fair and other events.

    + Works in partnership with Communications and Marketing on the development of Regional Center materials.

    Social Services

    + Helps to provide Social Services to individuals (and families/guardians) as needed by assisting with delivery of Social Services and assuring supportive services to individuals, families, guardians, and significant others.

    + Co-facilitation of the Speak Up for Friends (SUFF) group for the three Regional Centers.

    + Collaborates and develops necessary social work interventions within the ID team for delivery of services and supports.

    + Liaison with mental health agencies, CMAs, and advocacy agencies to ensure individuals’ rights are honored.

    + Provides advocacy services, informed consent, confidentiality and conflict resolution. This will include: working with guardians/parents and family members to address their issues and to provide training and assistance in ensuring individual rights are protected, implementation of grievance processes, transition planning, and obtaining benefits as needed.

    + Assists with supporting parents/guardians, when needed, in regard to Burial Planning, Funeral Planning, Cremation Planning, Advance Directives, setting up Disability Trusts, and other social service community resources.

    + Provides information related to the above.

    Minimum Qualifications, Substitutions, Conditions of Employment & Appeal Rights

     

    Mon-Fri 8-5 PM (work from home 3 days/office 2 days) (Subject to change)

    Experience Only:

    Six (6) years of relevant experience in an occupation related to the work assigned to this position

    OR

    Education and Experience:

    A combination of related education and/or relevant experience in an occupation related to the work assigned equal to six (6) years.

     

    Employment history is calculated on a full-time basis (40/hrs per week). Part-time employment is calculated on a prorated basis to determine qualifications.     Preferred Qualifications:

    + Bachelor’s degree in a human services related field

    + Experience working with individuals with intellectual and developmental disabilities (IDD).

    + Experience working with community IDD systems (i.e., CMA, PASAs, etc)

    + Experience working as part of a team

    + Relevant years of State service experience

    Conditions of Employment:

    + CDHS employees (all Direct contact with vulnerable persons): CBI name check and fingerprint check, ICON Colorado court database, Medicare fraud database, Reference Checks, Professional License verification (licensure requirements), Trails check (direct contact with children), CAPS (direct contact with adults – Mental Health Institutes, Regional Centers, Veterans Community Living Centers).

    + Travel – Travel is required for meetings, visiting other regional centers and training. If a State vehicle is unavailable, must have his/her own vehicle for transport. May submit for mileage reimbursement.
