    Responsibilities:

    GENERAL PURPOSE

    Demonstrates advanced knowledge of educational and community resources in assisting young adults with eating disorders and their siblings. Assists young adults with eating disorders to identify and meet needs and goals for recovery and life and works with the treatment team to refer them to other community resources as needed. Assist siblings to identify needs and goals related to supporting their loved one with an eating disorder and works with the treatment team to refer them to other community resources. Will assist siblings with skill building related to supporting loved ones at mealtime and performance of other caregiver behaviors supportive of recovery. Will engage in advocacy and service referral in collaboration with the WNYCCCED Care Managers. Collaborates with the Care Managers and Parent Peer Mentor to identify young adults/families at risk, e.g., those struggling with learning and implementing recovery-related skills, especially during transitions (e.g., going to college, moving from one level of care to the next, starting new employment, obtaining housing to begin independent living) and those not engaging in or not remaining engaged in treatment. Works with the treatment team to identify young adult/sibling strengths and alleviate barriers. Collaborates with the care managers and Parent Peer Mentor to assist with transition among levels of care to ensure individualized care and continuity of care. Also collaborates with the WNYCCCED Care Managers, Parent Peer Mentor, Director and Medical Director to offer community-based caregiver workshops and other educational (e.g., Project ECHO for Eating Disorders)/outreach events.

     

    SUPERVISION/DIRECTION RECEIVED; Reports to the Director of the WNYCCCED. Collaborates closely with the WNYCCCED care managers and Parent Peer Mentor and works with parents, family members and clients within a treatment team model.

    ESSENTIAL FUNCTIONS

    + Utilize advanced knowledge of educational and community resources and assist young adults with eating disorders to identify and meet needs and goals related to life and recovery to ensure ongoing recovery and prevent relapse.

    + Provide mentoring to young adults related to how to navigate transitions and ask for help

    + Assist young adult to identify self-care and relapse prevention strategies to promote continued motivation for change and strengthen relationships between patient, siblings and other family members.

    + Provide education and role modeling with opportunity for return demonstration regarding performance of other young adult behaviors facilitating recovery

    + Guide and assist siblings to identify and meet needs and goals related to supporting their loved one with an eating disorder and engage in teaching and skill building supportive of recovery skills to ensure ongoing recovery and prevent relapse.

    + Provide mentoring to siblings related to how to engage with their loved one to promote patient motivation and strengthen relationships between patient and siblings.

    + Teach siblings ways to support their loved one before, during and after mealtimes

    + Provide education and role modeling with opportunity for return demonstration regarding performance of other caregiver behaviors facilitating recovery

    + Collaborate with the care managers, parent peer mentor, and family to ensure that he young adult receives recovery-oriented support at mealtimes at home, school and/or work in order to ensure coordinated and continuous care and ongoing recovery.

    + Collaborate with the WNYCCCED Care Managers and Parent Peer Mentor to identify young adults/families at risk, e.g., those struggling with learning and implementing recovery-related skills, especially during transitions (e.g., going to college, moving from one level of care to the next, starting new employment, obtaining housing to begin independent living) and those not engaging in or not remaining engaged in treatment.

    + Works with the treatment team to develop strategies that alleviate barriers

    + Collaborates with the WNYCCCED Care Managers, Parent Peer Mentor, Director and Medical Director to develop and offer community-based caregiver workshops and other educational/outreach events including Project ECHO for Eating Disorders

    + Collaborates with the WNYCCCED Care Managers, Parent Peer Mentor, and treatment team to identify needs of young adult, siblings and other family members, as well as strengths and transition among levels of care to ensure individualized, coordinated and continuous care.

    + Independently navigate and engage in advocacy and service referral in collaboration with the WNYCCCED Care Managers (technical associate) and Parent Peer Mentor.

    + Develops and maintains good relationships with other internal and external providers to ensure continuity of care for patients and families within the Western New York Comprehensive Care Center for Eating Disorders.

    + Reports behavioral change to the treatment team and assists with intervention as directed in order to promote ongoing recovery and relapse prevention.

    + Participates in and incorporates professional supervision, team conferencing, and ongoing education toward quality care.

    + Maintains necessary records to meet grant, agency, and team requirements.

     

    Other duties as assigned

    MINIMUM EDUCATION & EXPERIENCE

    + Bachelor's degree in healthcare related field required.

    + 1 year experience in community health, human services, and/or patient liaison/advocate roles preferred.

    + Strong preference for young adults well-recovered from an eating disorder. Training in peer mentoring and/or previous supervised experience working in this role preferred.

    + Or equivalent combination of education and experience required.

    KNOWLEDGE, SKILLS AND ABILITIES

    + PC knowledge of Microsoft Word and Excel; email system, and clinical documentation system required.

    • **LICENSES AND CERTIFICATIONS**

    + A valid New York State Driver's License, have a satisfactory driving record and have access to reliable transportation.

    + Must possess age competencies to work with patients 12 years and older and their families required.
