**Work Schedule:** 8 Hr (8:00:00 AM - 5:30:00 PM)

**Pay Range:** $20.19 - $31.80 per hour

Summary:
The Patient Navigator will develop relationships with patients who are seeking Mohs Surgery. The Patient Navigator works with the Clinical and Administrative leadership to assist patients in scheduling, follow-up and execution of appointment scheduling, receipt of patient records and post care follow up. The Patient Navigator is responsible for conducting initial intakes for patients ensuring that all required documentation is in place prior to surgical scheduling.

Required Job Qualifications:
• High School Diploma or equivalent.
• Must be comfortable with outreach activities in unstructured environment
• Must be prepared to work occasional offsite events.
• Ability to work with culturally, racially and ethnically diverse clients and staff
• Excellent interpersonal and organizational skills and moderate computer skills
Preferred Job Qualifications:
• Personal experience navigating the healthcare system.
• Working knowledge of the target communities
• Bilingual English/Spanish
Responsibilities:
1. Initiates immediate contact with patients who reach out to Dermatology or are referred by other providers
2. Reminds patients of appointments and assists with appointment instructions and post op instructions
3. Follows-up to assist patients in scheduling follow-up visits.
4. Communicates with leadership about individual client's needs and concerns.
5. Shares knowledge and education to improve patients' ability to navigate services and systems.
6. Completes all documentation and reports in a timely manner.
7. Assists with coordination of and actively participates in scheduled educational and community forums.
8. Participates in all administrative, staff development and staff meetings.
