Description
General Description
Under general supervision, provides non-medical home support and transportation to program clients as needed. Effectively supports ongoing and collaborative communication amongst all program staff and is responsible for ensuring a trauma-informed care environment.
Essential Duties and Responsibilities
+ The priority or frequency of transportation vs in-home support will vary per program position (home cost center). Each staff person might be requested to be back up to a different program and to be flexible to fill in work time to give our participants the best support possible.
+ Transport clients to necessary appointments providing advocacy and support as needed.
+ Provide client assistance with grocery shopping, prescription pick-up, banking, friendly visiting, essential errands and light chores. May provide housekeeping services to clients such as make and change beds; dusting, vacuuming, light cleaning of kitchen, bedroom, bathroom; dishwashing; preparing light meals; making a list of needed cleaning items or grocery items; laundering, ironing; grocery shopping.
+ Must be able to lift and carry at least 20 pounds and use stairs.
+ Provides supervision services to clients (respite for caregivers). Monitor client’s safety.
+ Manages client contributions as needed.
+ Confirm rides/service with clients 24 hours prior to providing service.
+ Schedules appointments and necessary follow-up for assigned clients as needed.
+ Responds to calls from assigned clients within designated timeframes.
+ Keep clients apprised of available community resources and facilitates referrals.
+ Maintains accurate activity records.
+ Follows established procedures and policies for programs.
+ Keeps accurate mileage and timesheet records.
+ Maintains an up-to-date client service schedule on agency system (such as Outlook).
+ Maintain client service logs and accurately records trips and contacts and submits unit reports to supervisor monthly/weekly depending on the program.
+ Report problems, incidents to supervisor in a timely manner.
+ Keep safe and accurate records concerning receipt of cash/checks from clients.
+ Updates program staff/supervisor of any changes to client’s demographic information as soon as they are made aware.
+ Assist program management staff in completion of required clerical and/or administrative work as needed.
+ Maintain ongoing communication between clients and their service providers.
+ Ensure personal vehicle up-keep and repair to include, but not limited to, inspections and oil changes.
+ Complete annual or triennial defensive driving course.
+ Attends program, department, agency meetings and or trainings, as requested.
+ Awareness of and active support of the Agency's Mission, Vision, Values and Strategic Plan. Including, but not limited to, supporting the Agency's Core Competency initiatives of being Collaborative and
+ Community Focused, Innovative and Entrepreneurial, and Culturally Competent and Diverse.
+ Other duties as assigned.

Qualifications
Qualifications:
**Education:** High School diploma or equivalent.
**Credentials:** Possession of a valid NYS Driver’s license and use of a registered, insured, and reliable vehicle. Cars with four doors and or small SUVs preferred.
**Experience:** One year of driving experience and familiarity with city of Rochester and surrounding areas.
Bi-lingual skill to support client population REQUIRED (Spanish, Ukrainian, Dari, Swahili, Karen, or Nepali)
