Responsibilities:
GENERAL PURPOSE
This job consists of administrative and supervisory management of clinic operations pertaining to patient coordination within the service and broader UR Medicine system for all new internal and external requests for services to Pediatric Behavioral Health & Wellness ambulatory care. which The service receives approximately 5,000 referrals/year, provides approximately 65,000 patient visits/year, in 17 locations. This position involves extensive coordination with acute and ambulatory services within UR Medicine (e.g., specialty pediatric services, other psychiatric specialties) for overall service functioning. Oversight of existing and new processes regarding patient care/access for clinician transitions and for staff performing integral intake duties, including recruiting, orienting, and training Intake Team staff. The job involves extensive data collection, tracking, and interpretation via system reporting to develop management practices and control systems to ensure appropriate access and response for patient care, as well as utilization and management of service resources. Creates documentation processes and materials to reflect expanding programs and needs of the service; tTrain staff on use and implementation of processes and materials which contributes to consistent patient care access and management of resources. It involves communication via phone, MyChart, letters and inbasket messages with patients, families, referring clinicians/providers, and staff. The job requires a high level of competency, independent critical thinking, and problem-solving ability.
ESSENTIAL FUNCTIONS
Oversight and Supervision of Triage Functions
+ Organize, summarize, and triage requests for services from multiple internal and external sources (e.g., phone screens) with relevant clinical data/referral info/recommendations
+ Contact relevant clinicians/referring providers/specialty departments
+ Train and support screeners, provide feedback/oversee safety and CPS related concerns
+ Sign off on documentation when appropriate
+ Coordinate completion of all phone screens and screener schedules to ensure timely access for potential patients and their families; oversee appropriate dispositions
+ Assignment of cases in collaboration with Director or designee
+ Coordinate Child-Adult Transition cases with Strong Minds, Strong Ties, and Strong Recovery and support cases needing general adult transitions outside of UR system in collaboration with case management team and clinicians/providers to identify and establish adult care
+ Lead or co-lead Intake Team meetings (2 times/week) and provide weekly supervision to team members
+ Serves as a focal point for answering questions and solving problems regarding intake issues for difficult or unusual circumstances, assists in problem identification and resolution including direct communication with patient families
Systemic Coordination
+ Communication with PBHW staff, other departments, and external referring providers as appropriate to facilitate intake process/understanding/outcomes and coordination for cases. Including supervision of intake staff managing work queue and collaborating with enterprise staff managing work queue processes
+ Support coordination between higher levels of care including Child Psychiatry Partial and Inpatient services as well as Mobile Crisis Team, Behavioral Health Crisis Call Line.
+ Coordinate with medical director on specific and general intake matters
+ Support documentation creation/updates and processes related to psychiatric services and coordination of care within service
+ Coordination with Access Center (aka Call Center)
+ addressing issues as needed and attending monthly meetings to develop/update protocols and manage problem trends
Administrative Oversight, Data Management, and Resource Development
+ Utilize reporting for a variety of procedures
+ Weekly reconciliation to ensure no gaps in care through intake process
+ Monthly and annual compilation of data regarding all incoming referrals and tracking special projects to help further clinic understanding of trends/needs/resource management. Analyze and present data as needed.
+ Intermittent review of reports to ensure appropriate chart closures/attendance outreach and supervisory follow-up
+ Documentation Creation/Maintenance
+ Updating and creating any documentation service requires for intake procedures and otherwise (e.g. attendance, transfer process)
+ Update manual and train staff as needed on changes
+ Collaboration with Case Management team to identify and refine community resources with developing needs
Oversee caseload management:
+ Coordinate, in conjunction with primary clinical supervisors, caseload planning for trainees/staff leaving service, episodes of leave, transitioning within service
+ Create/maintain documentation/process and guide this process with collaboration of supervisors
+ Prepare schedules/caseloads for return from leave
+ Facilitate appropriate communication between patient families and clinicians/providers regarding limit setting and clinic policies
+ Provides feedback to clinicians and supervisors regarding caseload management/planning as it intersects with intake processes
Other duties as assigned.
MINIMUM EDUCATION & EXPERIENCE
+ Bachelor’s degree or an equivalent combination of education and experience required
+ Proficient computer experience (with programs such as Excel) required
