    Full-Time GIP Hospice Admissions/Transitional Care Registered Nurse (RN)

    • About the Role:

    As a Hospice RN, you’ll support patients and families by:

    - Evaluating and admitting GIP (General Inpatient) patients into hospice care.

     

    - Providing education to patients, families, and hospital staff about the hospice benefit and end-of-life care.

     

    - Collaborating with hospital staff and our business development team to build lasting relationships within Inspira Medical Centers.

     

    - Ensuring compassionate, high-quality care that honors the dignity and comfort of every patient.

     

    -Caring for clients in the inpatient setting, as well as facilities and/or homes.

     

    • Schedule: Full-Time | Monday–Friday | 10:00 AM – 6:30 PM

    • Territory: Gloucester, Salem, and Cumberland Counties, NJ

    • Key Responsibilities:

     

    - Complete GIP hospice admissions and evaluations within hospital settings

     

    - Provide patient-centered care aligned with the hospice plan

     

    - Administer medications, perform treatments, and manage wound care

     

    - Communicate changes in patient conditions to the care team

     

    - Document assessments and interventions using EMR on a mobile tablet

    - Serve as a resource for patients and families navigating the hospice journey

    • Qualifications:

     

    - Current RN license in New Jersey

     

    - Minimum 1 year of recent, hands-on clinical experience (hospice strongly preferred)

     

    - Graduate of an accredited nursing program

     

    - Solid communication, organization, and interpersonal skills

     

    - Ability to work independently and travel as needed

    - EMR experience preferred

    • **Additional Information:**

    _Base Pay, depending upon qualifications and subject to negotiation:_

    _$42.00 - $47.00 per hour_
