    Job Description:

    • Hospice Certified Nursing Assistant (CNA)

    •** **_Territory: Genesee County_** **| 🕒** **_Full-Time_** **| 🚗** **_Field-Based_

    •** **What You’ll Do as a Hospice CNA:

    + Provide personal care and support based on the patient’s Plan of Care, under the direction of a Registered Nurse.

    + Assist with daily living activities that bring dignity and comfort.

    + Monitor patient condition and report changes to the care team.

    + Offer compassionate emotional support to patients and families.

    + Help create a safe, supportive, and respectful home environment.

    + Collaborate closely with RN Case Managers to ensure quality, coordinated care.

    + Advocate for patients and help prevent avoidable hospitalizations.

    What You’ll Need:

    + High School Diploma or GED

    + CNA certification

    + At least **1 year of hospice or CNA experience**

    + Valid driver’s license, auto insurance, and reliable transportation

    + Ability to travel within **Genesee County** (approx. 50%)

    + Able to lift/move 50–100 lbs and meet physical demands of patient care
