•** **What You’ll Do as a Hospice LPN:
+ Administer prescribed medications to manage pain and symptoms
+ Monitor vital signs and overall patient condition, reporting changes to the RN or physician
+ Provide hands-on personal care: bathing, hygiene, dressing, mobility
+ Deliver comfort care measures to maintain dignity and quality of life
+ Accurately document care provided, symptoms observed, and patient status
+ Offer emotional support to patients and families, fostering trust and reassurance
+ Collaborate with RNs, physicians, chaplains, and social workers to deliver coordinated, holistic care
What You’ll Need:
+ Active LPN license (state-specific)
+ CPR certification (American Heart Association or Red Cross)
+ 1+ year experience in acute care or equivalent setting (hospice/home health preferred)
+ Strong communication, compassion, and clinical judgment
+ Reliable transportation and willingness to travel within **Livingston & Genesee Counties**
+ Positive attitude and passion for patient-centered care
