    Hours: M-F 9:30 am to 6 pm

     

    Salary: $90,000

     

    Pay to Stay Bonus: $7,500

    OVERVIEW OF PRIMARY RESPONSIBILITES :

    We are looking for a dynamic Youth Assertive Community Treatment (ACT) Clinical Team Leader who is passionate about working closely with youth ages 10-21 with acute behavioral health needs . The Youth ACT Team is a program designed to address the significant needs of children who are at risk of entering, or returning home from high intensity services, such as inpatient settings or residential services, through the use of a multi-disciplinary team. Youth ACT services are delivered in the home or other community based settings. Youth ACT teams deliver intensive, highly coordinated, individualized services and skilled therapeutic interventions through an integrated, multidisciplinary team approach to better achieve success and maintain the child in the home, school and community.

     

    The Clinical Team Leader is responsible for providing staff and programmatic supervision and needed, clinical services and ensuring that the Youth ACT team follows the mission of providing the child and their family with the level of support services and access to clinical professionals they require to sustain any gains made in crisis response or other out of-home high-intensity services.

    PRIMARY FUNCTIONS :

    I nclude coordination to improve quality of care through the efficient use of resources and thereby enhancing quality, cost-effective outcomes.

    + Lead staff meetings, clinical case conferences, and other agency or program meetings, including case presentations and conferences with other service providers/schools, family members, etc., as necessary.

    + Supervise and/or provide crisis intervention and other services for participants.

    + Assign and participate in in-service trainings, workshops, webinars, and conferences

    + Represent program at internal agency committees and meetings

    + Represent program to outside organizations.

    + Conduct consumer satisfaction surveys

    + Lead organizational and service planning meetings

    + Assign cases to staff

    + Provide clinical direction and supervision to staff

    + Provide quality control and assure adherence to program regulations and guidelines

    + Ensure that program integrates best practices and achieves its goals and objectives.

    + Develop and supervise schedules to meet service needs of participants and achieve optimal attendance and participation rates.

    + Ensure that programs are in compliance with funding sources and NYS licensing regulations.

    + Develop systems to track and monitor productivity on regular basis, and develop plans to address any decline in productivity.

    + Remain current on policies and procedures of Medicaid, managed care Medicaid, Medicare and insurance companies to facilitate coordination of program’s financial responsibilities.

    + Review and troubleshoot electronic health reporting errors (suspended services, overlaps, omissions, etc.) to ensure provided services are correctly entered for billing.

    + Monitor program expenditures.

    + Promote staff development in evidence-based practices, and spend 50% of his or her time in the field providing direct services to program recipients in their homes or at other community locations.

    ADDITIONAL FUNCTIONAL / ORGANIZATIONAL SUPPORT:

    + Train and supervise clinical and administrative staff including annual evaluations and supervisory notes, and approving clinical documentation of supervisees, as necessary.

    + Assure timely and satisfactory completion by self and supervisees of documentation in CAIRS and in electronic charts and case records.

    + Oversee and/or conduct intakes and complete admissions documentation.

    + Intervene with staff and participants in special situations

    + Other duties, as assigned.

    + Responsible for creating an on-call after hours rotating schedule and ability to provide 24/7 on-call and/or back-up on call, per rotating schedule.

    QUALIFICATIONS AND ATTRIBUTES :

    + Demonstrated knowledge of Ulster Counties services systems.

    + Ability to manage conflict, stress and multiple simultaneous work demands in an effective and professional manner.

    + Ability to work well independently, while collaborating with other team members.

    + Ability and willingness to self-motivate, to prioritize and change processes to improve effectiveness and efficiency.

    + Ability to adapt to changing program or organizational priorities.

    + Excellent writing skills.

    + Computer literacy. Ability to navigate Electronic Health Records and other systems.

    + Bi-lingual in English and Spanish, preferred.

    EDUCATION AND EXPERIENCE :

    + LCSW or LMHC required, LMSWs/LMHC-LPs will be considered based on experience in acute behavioral health settings and eligibility for obtaining full clinical credentialing within a limited amount of time.

    + 2+ years of supervisory experience.
