The anticipated wage for this position is between $62 and $64. Hourly wage may depend upon experience, education, geographic location and other factors.

**Job Title:** Licensed Physical Therapist – Home Health Care

**Location:** Baltimore City / Baltimore County (Nearby areas may be assigned based on patient availability)

**Weekly Commitment:** 30 units per week

Requirements:

+ Active Physical Therapist license in the state of Maryland

+ Experience in home health care

+ Proficiency with OASIS documentation

Preferred:

+ Experience using HCHB (Homecare Homebase / Point of Care EMR system)

Job Summary:

We are seeking a dedicated and skilled Physical Therapist to join our Clients home health care team. The therapist will perform evaluations and provide physical therapy treatment in accordance with physician orders and established care plans. Responsibilities include conducting comprehensive patient assessments using appropriate tools, reviewing medical records, establishing measurable goals, and developing individualized treatment plans aimed at maximizing patient independence and function.

The Physical Therapist will regularly reassess and update treatment goals, communicate effectively with physicians, care teams, patients, and families, and recommend adaptive equipment as needed. Accurate and timely documentation of evaluations, treatments, and billing is required, in compliance with company policies, HIPAA regulations, and all applicable state and federal laws.

Additional responsibilities include supervision of Physical Therapist Assistants (PTAs) and Physical Therapy Aides in accordance with state practice regulations. Participation in staff development, training sessions, and ongoing continuing education is expected to maintain and enhance clinical skills.

**Pay Details:** $62.00 to $64.00 per hour
