    The anticipated wage for this position is between $62 and $64. Hourly wage may depend upon experience, education, geographic location and other factors.

     

    **Job Title:** Physical Therapist – Home Health

    **Location:** Washington County, Maryland

    **Schedule:** Monday–Friday, 8:00 AM – 5:00 PM

     

    No weekends, holidays, or on-call required

    Position Summary:

    We are seeking a licensed and experienced **Physical Therapist** to provide in-home physical therapy services across **Washington County, MD** . This role includes conducting comprehensive assessments, developing and implementing individualized treatment plans, and overseeing the care provided by Physical Therapist Assistants (PTAs). The ideal candidate will be skilled in **OASIS documentation** and have experience in home health settings. Familiarity with **HomeCare HomeBase (HCHB)** is preferred.

    Key Responsibilities:

    + Conduct in-home physical therapy evaluations and treatments in accordance with physician orders.

    + Utilize appropriate assessment tools to complete comprehensive patient evaluations.

    + Review medical records to assess diagnoses, history, precautions, and contraindications.

    + Develop measurable goals and treatment plans to support optimal patient function and independence.

    + Monitor and reassess treatment effectiveness, adjusting plans as needed.

    + Communicate patient progress and needs to physicians, care teams, patients, and families.

    + Recommend and assist with adaptive equipment to enhance rehabilitation outcomes.

    + Accurately document all evaluations, treatments, and billing in compliance with company, regulatory, and legal standards.

    + Supervise PTAs and Physical Therapy Aides in accordance with Maryland state practice regulations.

    + Participate in staff training, development programs, and continuing education as required.

    Qualifications:

    + Active **Maryland Physical Therapist license**

    + Experience in **home health** and **OASIS documentation**

    + Knowledge of or experience with **HomeCare HomeBase (HCHB)** is a plus

    **Pay Details:** $62.00 to $64.00 per hour
