Responsibilities for a social worker

+ Assists residents with emotional adjustment to facility.

+ As directed, interview new residents and records, social history, and assessments.

+ Completes the social services portion of the MDS, Care planning, baseline, etc.

+ Refers to psychological services as needed.

+ Schedules and attends and participates in interdisciplinary care plan meetings morning meetings, utilization review, etc.

+ Writes summaries and progress notes as directed.

+ Assists in conducting a pre-discharge evaluation including but not limited to setting up Visiting Nursing Services, resources, etc. in the community.

+ Assists with discharge planning. Follows up after discharge.

+ Conducts required Cut Letters

+ Writes discharge summaries as assigned.

+ Develops a professional relationship with each resident and family.

+ Educates residents/ representatives on Advanced Directives.

+ Adheres to all Social Services Regulations.

Qualifications for a social worker

+ Minimum Bachelor's Degree in Social Work from an accredited College.

+ Must obtain a CSW and/or LSW.

+ One to Three years of professional experience and/or training; or equivalent combination of education and experience.
