UPMC Western Maryland is seeking a Full-Time Service Coordinator!
This is a full-time, dayshift position. Candidate must be comfortable meeting patients in a community setting, including the patient's home.
The Service Coordinator provides community resource and family support service coordination to patients and families, according to the level of need. The primary function of the Service Coordinator is to provide patient-families with professional assessment, service planning, service coordination, referral, and re-evaluation services required for a safe and healthy community life, which is manifested through stability in health-related social needs. Approximately 50% of this role is patient and family-facing, and approximately 50% is program coordination and facilitation.
Responsibilities:
+ Coordination & Support: Provides professional coordination of community resources and family support services based on individual needs, promoting health and well-being.
+ Assessment: Evaluates individual and family strengths and needs through interviews and reviews, maintaining a trusting relationship and reassessing regularly.
+ Service Planning: Collaborates with patients, families, and service providers to develop, document, and implement comprehensive service plans.
+ Resource Utilization: Ensures needs are met through natural supports, community services, and specialized services, helping individuals and families access and coordinate these resources.
+ Resource Investigation: Identifies new resources, liaises with directors, and maintains an updated catalog of community resources.
+ Advocacy: Advocates for individuals to ensure responsiveness from various services, providing information, removing barriers, and resolving problems.
+ Record Keeping: Maintains accurate records of service coordination activities, updates forms, reviews charts for compliance, and prepares for audits.
+ Primary Resource: Acts as the go-to resource for individuals and their families, ensuring effective safety net resources and cross-system coordination.
+ Agency Cooperation: Fosters a cooperative agency culture, focusing on problem resolution, communication, and participation in special projects.
+ Bachelor's Degree in a Human Services field such as Social Work, Psychology or Counseling required. Other fields of study such as Public Health or Education may be considered with coursework of at least 12 credits in Human Services.
+ One-year related experience preferred.
+ Prior experience working in community settings preferred.
+ Safe driving record required. **Licensure, Certifications, and Clearances:**
+ Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)
+ Driver's License
