Job Summary: The Population Health Coordinator will process coordinating care, reviewing patient details and handling medical record management. The role will function as clinical extender to expand departmental capacity for proactive patient outreach to improve outcomes, reduce utilization, decrease costs, and improve patient satisfaction.
Minimum Education: High School diploma or GED.
Licensure, Registration and/or Certification: Certified Medical Assistant, preferred.
Work Experience: Minimum 3 years of related experience in care delivery and coordination in a healthcare setting. Medical office experience, preferred
Knowledge, Skills and Abilities: Effective interpersonal, written and oral communications. Demonstrated computer skills. EPIC and EMR knowledge, preferred. Effective organization for a multi-task role. Ability to work in a team environment.
Essential Functions and Responsibilities: Performs telephonic outreach to identified patient populations strictly utilizing scripting, decision trees, or existing nurse-documented care plans. Promotes patient wellness and creates a sense of accountability by reinforcing established care plan at RN determined intervals. Escalates any assessment, teaching, referrals or follow ups to the appropriate LPN or RN, as needed. Documents all outreach in EPIC utilizing approved template. Maintains medical records by scanning and correctly entering external results to achieve quality measure attainment. Assists in coordination of care through patient messaging, telephone calls, scheduling appointments, coordinating transportation, assisting with the completion of forms, prior authorizations, and etc. according to established protocols. Successfully uses the EMR and other computer applications to accomplish tasks according to policies and procedures. Tracks ordered or pending tests, procedures and referrals according to protocol. Schedules tests and appointments for referred services. Participates in quality initiatives and safety functions. Remains informed and compliant with current Health System, Warren Clinic, OSHA and HIPAA policies and procedures. Maintains exemplary patient satisfaction and participates as an engaged team member.
Decision Making: Independent judgment in making minor decisions where alternatives are limited and standard policies/protocols have been established.
Working Relationships: Works directly with patients and/or customers. Works with internal/external customers via telephone or face to face interaction. Works with other healthcare professionals and staff.
