The Heart Transplant Coordinator oversees the multidisciplinary team in all aspects of pre and post transplant care in both inpatient and outpatient clinical settings and can assume clinical duties. The Heart Transplant Coordinator works directly with the Medical Director, the lead Heart Failure attendings, the Director of external transplant program partnerships, and the Director of Cardiogenic Shock and the lead Heart Failure NP to oversee and drive processes to improve patient outcomes. The Heart Transplant coordinator promotes collegiality within the multiple departments, internal and external clinical partners in caring for the heart transplant patients including education, process improvement, delivery of care and data abstraction.
POSITION RESPONSIBILITIES
+ Assist with heart transplant initiatives – including but not limited to: oversight of transplant protocol implementation, clinical follow through on pre and post transplant imaging, lab results, complications and mortality rates, reduction of admission, optimize transitions of care.
+ Work with administrative director to oversee inpatient and outpatient Heart Transplant programs, provide data and insight into program growth and quality
+ Represent the heart transplant program and its activities within the AHS system and hospital level committees as needed
+ Develop and maintain relationships with clinical subspecialty partners who have advanced knowledge in caring for post transplant patients.
+ Work collaboratively with a variety of professional, administrative and community leaders to raise awareness of heart transplant patient care and needs.
+ Improve operational efficiency of pre-transplant patient workup and post transplant patient care including maintenance of computer-based reporting and data

ESSENTIAL SKILLS AND EXPERIENCE
+ Licensed Advanced Practice Provider from an accredited college or university
+ Ability to communicate clearly and in a supportive manner with patients and/or families regarding condition, prognosis, health needs and available services
+ Extensive knowledge of current trends and developments in the care of Heart Transplant patients
+ Ability to develop and maintain effective working relationships with professional staff, local officials, and the public.
+ Keeps abreast of current research and incorporates relevant findings into specific areas of clinical practice regarding the care of the Heart Transplant patients
