    Job Description:

    Phoenix Home Care & Hospice is seeking compassionate LPNs and RNs to provide one-on-one care in the homes of our clients. We prioritize your professional satisfaction and personal well-being, so you can focus on delivering the highest quality care.

    Responsibilities:

    + Follow an individualized plan of care as directed by the physician

    + Monitor and document patient condition and care provided

    + Provide education and support to clients and families

    + Complete electronic charting and documentation

    + Participate in training sessions and staff meetings

    Requirements:

    + Active LPN or RN license (State of Missouri or compact)

    + Strong communication and problem-solving skills

    + Professional, positive, and flexible attitude
