    Job Description:

    Phoenix Home Care & Hospice is hiring Licensed Practical Nurses (LPNs) and Registered Nurses (RNs) to provide 1:1 skilled nursing care in the homes of our clients in Mt. Vernon, MO.     Job Responsibilities:

    + Follow individualized plans of care developed in coordination with physicians

    + Monitor, document, and report patient conditions using electronic charting

    + Educate and support clients and their families

    + Collaborate with the care team and attend staff meetings as needed

    Qualifications:

    + Active State of Missouri or Compact Nursing License (LPN or RN)

    + Strong communication and critical thinking skills

    + Professional, positive, and compassionate attitude

    + Ability to work independently and manage time effectively
